Performer lists exist to provide the public, the NHS and the clinical professions with an assurance that GPs, dentists and opticians in NHS primary care are appropriately qualified, experienced and suitable for this work.
If you are a doctor, dentist or optician, you must be on a performer list in order to work in NHS primary care. You can be on one list only, this should be the one where you spend the most time working. You can apply for inclusion on the list of Hampshire, Southampton City, Portsmouth City or Isle of Wight PCTs using the forms downloadable from this page.
Your original documents and proof of ID will need to be verified in person, so you should make an appointment to come to our office in Winchester. We can also help you to apply for an enhanced criminal records bureau disclosure and, if you are a doctor, make sure that you have a smartcard.
You need to allow at least 12 weeks for these checks and for references to be obtained. Making an application is no guarantee that you will be included, so it is important that you do not start work until your inclusion has been confirmed.

All GPs, Dentists and Opticians are checked at the time when they appy for inclusion on a local performers list. You do not need to pay for this check, it is part of the application.
In addition, GPs, Dentists and Opticians from outside the UK are asked to provide Police checks from the country where they have lived and worked.
