Hampshire Community Health Care

The part of the Primary Care Trust that formerly provided community health services throughout Hampshire – including running most of its community hospitals became a separate organisation - Hampshire Community Health Care (HCHC) before joining with Hampshire Partnership Foundation Trust to become Southern Health NHS Foundation Trust.

Many of their services help people stay independent in their own homes, help reduce unnecessary admissions to large hospitals, provide support for patients after major operations and to children and their families. 

Southern Health provides many primary care services through hard-working teams in the community throughout Hampshire, including: 

  • For adults– podiatry, physiotherapy, occupational therapy, scheduled care (including Orthopaedic Choice), inpatient services at community hospitals, minor injuries units, community matrons and rapid response teams.
  • For children– health visiting, school nurses, children and adolescent mental health (in south-east Hampshire), safeguarding children, occupational therapy and physiotherapy
  • Hampshire’s stop smoking service, Quit4Life, the Chlamydia screening programme and other sexual health services.
