From 1 April 2009 to 31 March 2011, Hampshire Primary Care Trust operated as two separate autonomous organisations in line with Department of requirements: Hampshire Community Health Care, which provided a range of community-based services to the people of Hampshire; and NHS Hampshire, which invested and commissions all the health care for the local population. These organisations were governed by the Hampshire Community Health Care Board and the NHS Hampshire (Commissioning) Board respectively.  

Hampshire Primary Care Trust remained the legal umbrella organisation for both organisations.
 From April 2011, services provided by Hampshire Community Health Care were integrated with those provided by Hampshire Partnership NHS Foundation Trust, in the organisation now known as Southern Health NHS Foundation Trust. 

During 2011/12, as part of the Government's plans to reform the NHS, the Primary Care Trusts serving Southampton Ciity, Hampshire, Isle of Wight and Portsmouth (SHIP) have been working as a cluster, in order to ensure resilience during the transition to clinical commissioning. Whilst the four PCTs remain as statutory bodies, since June 2011, they work together under a joint SHIP PCT Cluster Board, supported by a single management team.
